This webinar will explore the role of social prescribing link workers (SPLWs) in neighbourhood teams and integrated care. This session will focus on how link workers can strengthen collaboration, improve continuity of care and help connect people to the right support in their community. 

We will take a closer look at the unique contribution of SPLWs within neighbourhood health ecosystems, including how they help bridge the gap between clinical services and community-based provision. We will explore how link workers can build relationships with key partners, work effectively with stakeholders and clearly articulate their role and value to colleagues. 

Through practical insight and examples, this session will support participants to identify opportunities for stronger neighbourhood working and improved collaboration. This webinar will help SPLWs build confidence in their role within integrated care and strengthen their contribution to local neighbourhood teams. 

Learning outcomes:

By the end of this session, participants will be able to: 

  • Understand the unique contributions of Social Prescribing Link Workers and be confident in articulating this with colleagues
