How can we use data and evidence to identify where movement-based social prescribing is needed most, and how do we demonstrate the difference it makes? 

Different parts of the system hold different pieces of the picture. By connecting these insights we can better identify unmet need, target support proactively and understand what happens across the whole pathway. This session will explore how a more joined-up approach can strengthen impact measurement and help demonstrate value and return on investment. 

Designed primarily for social prescribers, primary care and those involved in local planning and service development, the session will also be relevant to Active Partnerships and physical activity providers.  

We'll look at: 

  • How evidence, population data and community insight can help identify people and communities who may benefit most from movement 
  • The different perspectives and information that Link Workers, primary care and physical activity providers can contribute 
  • Using feedback and data to understand what's working, identify unmet need and strengthen the case for investment 

The session will equip participants to make better use of the information and insight available within their own role, and understand how working with others can build a fuller picture of local needs, opportunities and impact.
