Discover how effectively capturing data can strengthen the impact of social prescribing in this practical and informative webinar. We will also introduce the different types of outcomes captured within the Common Outcomes Framework (COF) and explain how these contribute to building a clearer picture of impact. 

Participants will also gain practical insights into simple techniques for embedding data capture into appointments and follow-ups, along with guidance on recording information accurately and consistently within digital systems. Through practical examples and shared learning, the session aims to build confidence in collecting outcomes data in ways that feel meaningful for both practitioners and the people they support.
